Total T4 in µg/dL and nmol/L: Why the Same Result Looks Like Two Different Tests

Picture this. You had bloodwork done at home, and your Total T4 came back as 7.2 µg/dL. A few months later you’re overseas, get a repeat panel, and the result reads 93 nmol/L. Why Does T4 Look So Different Between the Two Units?

One measures weight per volume, the other measures particle count per volume, and the bridge between them is thyroxine’s molecular weight. Before You Convert: Make Sure You’re Actually Holding a Total T4 Result

This is the mistake that trips people up more than the math itself. Thyroid panels report several different things that all sound similar: Total T4, Free T4, Total T3, Free T3. Free T4 in particular uses entirely different units, usually pmol/L or ng/dL, and an entirely different conversion factor. If you run a Free T4 number through the Total T4 formula above, you’ll get a confidently wrong answer that looks perfectly reasonable. Check your report for the word “Total” or “Free” next to T4 before doing anything with the number.

Why the US Still Uses µg/dL
